目的探讨低位直肠癌前切除术后吻合口瘘的防治方法。方法回顾性分析14例低位直肠癌患者行前切除术后发生吻合口瘘的临床资料。结果吻合口瘘发生于术后5～14d,平均7d;均经综合治疗后痊愈,愈合时间8～12d;未行肠造瘘及2次手术。结论术前充分准备、术中严密操作可预防低位直肠癌前切除术后吻合口瘘发生;正确放置引流管及灌洗引流、有效控制感染并应用生长抑素等,有助于吻合口瘘愈合。
Objective To investigate the prevention and treatment of anastomotic fistula after resection of low rectal cancer. Methods Retrospective analysis of 14 cases of low rectal cancer patients undergoing anastomotic fistula clinical data. Results Anastomotic fistula occurred in 5 ~ 14 days postoperatively, with an average of 7 days. All patients were recovered after comprehensive treatment and the healing time was 8-12 days. No intestinal fistula and 2 surgeries were performed. Conclusions The preoperative preparation is adequate and strict intraoperative and operative management can prevent the anastomotic fistula after the resection of low rectal cancer. Correct fistula drainage and lavage drainage, effective infection control and the use of somatostatin are helpful for the healing of anastomotic fistula .
