目的比较高血压细节管理与分组管理实施1年后的效果。方法按照类实验设计,选取干预社区568名高血压患者,对照社区189名高血压患者为研究对象,干预组实施细节管理,对照组实施常规的分组管理。项目实施1年后,比较完成评估的508名干预组病人和178名对照组病人的管理效果。结果干预组在全家酱油食用量下降、舒张压下降、腰围下降、控制胆固醇升高、控制空腹血糖升高、控制血清肌酐升高6个方面的管理效果优于对照组(P均<0.05)。与干预组比较,对照组全家植物油食用量下降、高密度脂蛋白胆固醇升高、低密度脂蛋白胆固醇下降,差异有统计学意义(P均<0.05)。干预组每降低1 mm Hg血压值的成本低于对照组。结论高血压细节管理项目是社区高血压管理的一种有效策略,其长期效果和在社区的持续可操作性尚需进一步探索。
Objective To compare the effect of one-year follow-up of hypertension detail management and group management. Methods According to the experimental design, 568 hypertensive patients were selected in the intervention group and 189 hypertensive patients in the community were selected as study subjects. The intervention group was administered with detail management and the control group with routine group management. One year after the project was put into operation, 508 patients in the intervention group and 178 patients in the control group who completed the evaluation were compared. Results The intervention group had better management than the control group in reducing the consumption of soy sauce, decreasing diastolic blood pressure, lowering waist circumference, controlling cholesterol, controlling fasting blood glucose and controlling serum creatinine in the intervention group (P <0.05). Compared with the intervention group, the consumption of vegetable oil in the control group decreased, high-density lipoprotein cholesterol increased, low density lipoprotein cholesterol decreased, the difference was statistically significant (P all <0.05). The cost of lowering 1 mm Hg blood pressure in the intervention group was lower than that in the control group. Conclusion The Hypertension Management Project is an effective strategy for management of hypertension in the community. Its long-term effectiveness and continuing operability in the community need to be further explored.
