目的 检测口腔颌面部癌肿活检标本的体外化疗药物敏感性 ,明确临床常用各种化疗药物对口腔癌敏感性差别。方法 1 50例口腔癌化疗前活检新鲜组织标本行改良MTT药敏检测。检测用药是顺铂、5 氟尿嘧啶、平阳霉素、紫杉醇、替尼泊苷、表阿霉素、长春地辛和甲氨蝶呤 8种常用化疗药物。结果 1 50例标本成功完成MTT药敏检测的有 1 4 0例 ,检测成功率是 93 33 %。MTT药敏检测所用药物浓度是各药物峰血浆浓度的 5倍。结果表明 ,8种药物对口腔癌细胞体外平均生长抑制率分别是 :替尼泊苷 63 76 %、顺铂 2 5 93 %、表阿霉素 2 5 86 %、紫杉醇 2 3 52 %、平阳霉素 2 2 97%、氟尿嘧啶 2 2 0 8%、长春地辛 1 8 42 %、甲氨蝶呤 1 8 93 % ;替尼泊苷对口腔颌面恶性肿瘤体外抗瘤效果显著高于其他 7种化疗药物 (P <0 0 5)。 >40 %的口腔鳞状细胞癌标本对替尼泊苷、顺铂、表阿霉素 3种药物中度以上敏感 ;>40 %的腺源性口腔癌标本对替尼泊苷、紫杉醇和表阿霉素中度以上敏感。结论 大多数口腔颌面部癌肿对替尼泊苷敏感 ,顺铂、表阿霉素和紫杉醇敏感性也较高。替尼泊苷、表阿霉素和紫杉醇是口腔颌面部恶性肿瘤有潜力的化疗药物 ,其敏感性强于长春地辛、氟尿嘧啶和甲氨蝶呤。临床上应用改良MTT药敏检测法 ,筛选口腔颌面?
Objective To detect the sensitivity of chemotherapeutic agents in oral and maxillofacial biopsy specimens in vitro and determine the sensitivity of different chemotherapeutics to oral cancer. Methods A total of 50 specimens of oral cancer before biopsy were examined by modified MTT assay. Drug testing is cisplatin, 5 fluorouracil, Pingyangmycin, paclitaxel, teniposide, epirubicin, vindesine and methotrexate eight kinds of commonly used chemotherapy drugs. Results One hundred and fifty cases successfully completed the MTT susceptibility test with 140 cases and the detection success rate was 93.33%. MTT drug sensitivity test drug concentration is 5 times the peak plasma concentration of each drug. The results showed that the inhibitory rates of eight drugs against oral cancer cells in vitro were 63 76% of teniposide, 2593% of cisplatin, 2586% of epirubicin, 2352% of paclitaxel, 22 227% of the total, 2280% of the fluorouracil, 1842% of the vindesine and 1893% of the methotrexate. The in vitro anti-tumor effect of teniposide on oral and maxillofacial malignant tumors was significantly higher than that of the other 7 Chemotherapy drugs (P <0 05). > 40% of oral squamous cell carcinoma specimens were moderate to sensitive to teniposide, cisplatin and epirubicin;> 40% of oral adenocarcinoma specimens of teniposide, paclitaxel and Adriamycin is moderately sensitive. Conclusions Most oral and maxillofacial cancers are sensitive to teniposide, with higher sensitivity to cisplatin, epirubicin and paclitaxel. Teniposide, epirubicin and paclitaxel are potential chemotherapeutic drugs for oral and maxillofacial malignancies and are more sensitive than vindesine, fluorouracil and methotrexate.
