目的评价多层螺旋CT对脾实性肿瘤的诊断价值。资料与方法回顾性分析16例经病理、穿刺或手术证实的脾实性肿瘤患者的CT资料,分别由2名放射科医师采用双盲法对病灶的CT征象进行分析,最后统一意见判断其良、恶性。采用Fisher精确校正检验及t检验进行统计学分析。结果脾良、恶性肿瘤的大小、数目、形态及脾有无肿大无统计学差异(P>0.05),但患者症状、病灶有无环形强化及腹腔或后腹膜淋巴结有无肿大有统计学差异(P<0.05)。CT对良恶性诊断结果进行统计学分析,P<0.05,差异具有统计学意义。CT鉴别脾良、恶性肿瘤的敏感度、特异度、阳性预测值、阴性预测值分别为77.8%、85.7%、87.5%、75.0%。结论多层螺旋CT具有鉴别脾良、恶性肿瘤的诊断价值,使一些良性肿瘤患者避免不必要的切脾手术,帮助淋巴瘤患者及早明确治疗。
Objective To evaluate the diagnostic value of multislice spiral CT in splenic solid tumors. Materials and Methods The CT data of 16 patients with splenic solid tumors confirmed by pathology, puncture or operation were retrospectively analyzed. Two radiologists analyzed the CT signs of lesions by double-blind method, , Malignant. Fisher exact calibration test and t test were used for statistical analysis. Results There was no statistical difference in the size, number, shape and splenomegaly or splenomegaly (P> 0.05) between splenic benign and malignant tumors, but there was no statistically significant difference in symptoms, lesions with or without annular enhancement and intra-abdominal or retroperitoneal lymph node enlargement Difference (P <0.05). CT of benign and malignant diagnostic results were statistically analyzed, P <0.05, the difference was statistically significant. The sensitivity, specificity, positive predictive value and negative predictive value of CT in distinguishing benign and malignant spleen were 77.8%, 85.7%, 87.5% and 75.0%, respectively. Conclusion Multi-slice spiral CT has the value of differentiating between benign and malignant splenic tumors. It can prevent some benign tumor patients from unnecessary splenectomy and help patients with lymphoma to have definite treatment as soon as possible.
