目的:对比分析用开腹手术和用腹腔镜手术治疗直肠癌的可行性、安全性和肿瘤根除率。方法:采用随机法将我院2010年9月~2011年6月收治的68例直肠癌患者分为两组。其中一组为开腹手术组,开腹手术组患者接受开腹手术,另外一组为腹腔镜组,腹腔镜组患者接受腹腔镜手术。待治疗结束后,比较两组患者的手术时间、术中出血量、标本切除长度、淋巴结清扫个数、术后并发症发生率、术后肠道的恢复程度和住院时间。结果:①腹腔镜组患者在术中出血量、术后肠道功能的恢复时间和住院时间这三方面优于开腹手术组患者,且两组间的差异具有显著性(P<0.05);②腹腔镜组患者的术后并发症发生率低于开腹手术组患者的术后并发症发生率,且两组间的差异具有显著性(P<0.05);③两组患者在手术时间、标本切除长度和淋巴结清扫个数这三方面不具有显著性差异(P>0.05)。结论:腹腔镜手术治疗直肠癌不仅可以得到与开腹手术相一致的肿瘤治疗效果,还在手术创伤、术中出血量、患者住院时间、术后恢复速度和并发症发生等方面更优于开腹手术,是一种值得在临床上推广和应用的治疗直肠癌的好方法。
Objective: To compare the feasibility, safety, and tumor eradication rates of open rectal cancer with laparoscopic surgery and laparoscopic surgery. METHODS: Sixty-eight patients with rectal cancer who were admitted to our hospital from September 2010 to June 2011 were randomly divided into two groups. One group was open surgery group, laparotomy group received laparotomy, the other group was laparoscopic group, laparoscopic group patients underwent laparoscopic surgery. After the treatment was completed, the operative time, intraoperative blood loss, specimen removal length, number of lymph node dissections, incidence of postoperative complications, postoperative recovery of the intestinal tract, and length of stay were compared between the two groups. Results: 1 The laparoscopic group was superior to the open surgery group in terms of intraoperative blood loss, postoperative recovery time of intestine function and length of stay, and there was a significant difference between the two groups (P<0.05). 2The incidence of postoperative complications in patients of laparoscopic group was lower than that of patients in open surgery group, and the difference between the two groups was significant (P<0.05).3The operation time, There was no significant difference in the length of specimen removal and lymph node dissection (P>0.05). Conclusion: Laparoscopic surgery for rectal cancer can not only achieve the same therapeutic effect as open surgery, but also is superior to open surgery, intraoperative blood loss, hospital stay, postoperative recovery rate, and complications. Abdominal surgery is a good way to treat rectal cancer worthy of promotion and application in the clinic.
