目的探讨激素替代治疗对围绝经期综合征(MPS)妇女子宫内膜厚度和激素水平的影响。方法选取2013年2月-2014年2月间该院收治的MPS患者102例,按入院顺序随机分为观察组和对照组,每组各51例。对照组给予口服谷维素片治疗,观察组在此基础上加服倍美力和黄体酮胶丸,两组均治疗12周。分别于治疗前后检测两组子宫内膜厚度,黄体生成素(LH)、卵泡刺激素(FSH)、孕酮(P)和雌二醇(E2)水平;采用Kupperman评分法和汉密尔顿抑郁量表(HAMD)评价围绝经期症状和抑郁症改善情况。结果治疗前两组孕产妇子宫内膜厚度之间差异无统计学意义(P>0.05);治疗后观察组患者较对照组明显增厚,差异有统计学意义(P<0.05)。治疗前两组患者各激素指标之间差异均无统计学意义(P>0.05);治疗后两组患者LH、FSH水平均明显降低,P、E_2水平均明显升高,观察组改善幅度明显大于对照组,差异有统计学意义(P<0.05)。治疗前两组患者Kupperman评分和HAMD评分之间差异无统计学意义(P>0.05);治疗后两组患者Kupperman评分和HAMD评分均明显低于治疗前,观察组降低幅度明显大于对照组,差异有统计学意义(P<0.05)。结论激素替代治疗可明显增加MPS患者子宫内膜厚度,改善体内激素水平,缓解围绝经期症状和不良情绪。
Objective To investigate the effects of hormone replacement therapy on endometrial thickness and hormone levels in women with perimenopausal syndrome (MPS). Methods Totally 102 MPS patients admitted to our hospital from February 2013 to February 2014 were randomly divided into observation group and control group according to the order of admission, with 51 cases in each group. The control group was given orally administered oryzanol tablets, and the observation group was given Betametrex and progesterone capsules on the basis. Both groups were treated for 12 weeks. The levels of endometrial thickness, luteinizing hormone (LH), follicle stimulating hormone (FSH), progesterone (P) and estradiol (E2) were measured before and after treatment. The Kupperman score and Hamilton Depression Scale HAMD) to evaluate the improvement of menopausal symptoms and depression. Results There was no significant difference in the endometrial thickness between the two groups before treatment (P> 0.05). After treatment, the thickness of the endometrium in the observation group was significantly thicker than that in the control group (P <0.05). There was no significant difference between the two groups before treatment (P> 0.05). After treatment, the levels of LH and FSH in both groups were significantly decreased, the levels of P and E 2 were significantly increased, and the improvement in the observation group was significantly greater than Control group, the difference was statistically significant (P <0.05). There was no significant difference between the two groups before treatment Kupperman score and HAMD score (P> 0.05); Kupperman score and HAMD score were significantly lower in both groups after treatment than before treatment, the observation group decreased significantly more than the control group, the difference There was statistical significance (P <0.05). Conclusion Hormone replacement therapy can significantly increase the endometrial thickness in MPS patients, improve the hormone levels in the body and relieve perimenopausal symptoms and adverse emotions.
