目的应用Meta分析的方法分析HLA-DRB1基因多态性与幽门螺杆菌感染的相关性。方法检索中文数据库包括中国知网、万方、维普,英文数据库包括Pub Med、MEDLINE、OVID。含有HLA-DRB1、幽门螺杆菌等相关的文献。数据提取后,将H.pylori感染患者作为病例组,H.pylori未感染患者作为对照组,采用Rev Man5.0进行分析,效应指标采用OR值和95%CI,异质性大小以I2进行表示。进行亚组分析和敏感性检验,用Egger’s法通过Stata12.0定量评估发表偏倚。结果 HLA-DRB1*08二组比较,差异有统计学意义。HLA-DRB1*1502二组比较,差异有统计学意义。对HLA-DRB1*08按照地区(欧洲与中国)进行亚组分析,二组比较,欧洲地区差异无统计学意义,中国地区差异有统计学意义。而按照H.pylori检验方法进行亚组分析,HLA-DRB1*1501,二组比较,联合试验差异有统计学意义,单项试验差异无统计学意义。HLA-DRB1*11,二组比较,联合试验Z=1.97(P=0.05),单项试验差异无统计学意义。结论 HLA-DRB1*08和HLA-DRB1*1502是H.pylori感染的易感基因。其中HLA-DRB1*08是中国地区特有的H.pylori感染易感基因。H.pylori检验方法是否作为HLA-DRB1*1501、*11的影响因素值得进一步分析研究。
Objective To analyze the association between HLA-DRB1 gene polymorphism and Helicobacter pylori infection by Meta-analysis. Chinese database retrieval methods, including CNKI, Wanfang, VIP, English database, including Pub Med, MEDLINE, OVID. Contains HLA-DRB1, Helicobacter pylori and other related literature. After data extraction, patients with H.pylori infection were selected as the case group, and patients without H.pylori infection were used as the control group. The data were analyzed by using Rev Man5.0. OR was used as 95% CI and the effect size was represented by I2 . Subgroup analyzes and sensitivities were performed and publication bias was assessed quantitatively using Stata 12.0 using Egger’s method. Results HLA-DRB1 * 08 two groups, the difference was statistically significant. HLA-DRB1 * 1502 two groups, the difference was statistically significant. The subgroup analysis of HLA-DRB1 * 08 by region (Europe and China) showed no significant difference between the two groups in the two groups, and the difference in China was statistically significant. According to the H.pylori test, subgroup analysis showed that HLA-DRB1 * 1501 had no significant difference between the two groups in combination test. HLA-DRB1 * 11, the two groups, the joint test Z = 1.97 (P = 0.05), single test difference was not statistically significant. Conclusion HLA-DRB1 * 08 and HLA-DRB1 * 1502 are susceptible genes of H.pylori infection. Among them, HLA-DRB1 * 08 is a unique susceptibility gene of H.pylori infection in China. Whether H.pylori test as HLA-DRB1 * 1501, * 11 factors worthy of further analysis.
