目的分析声脉冲辐射力成像技术对慢性肝病肝纤维化的诊断价值。方法随机抽取2013年6月至2014年10月来我院检查的肝病患者作为研究对象,采用回顾分析法,将其分为慢性肝病组(60例)、肝硬化代偿期组(45例)、肝硬化失代偿期组(45例)、正常对照组(50例),对四组人员进行声脉冲辐射力成像技术(ARFI)检测,并评价四组检验人员的慢性肝病门静脉内径、门静脉流速和ARFI做统计分析比较。结果慢性肝病组的ARFI检测剪切波速度为(2.34±0.64)m/S,肝硬化代偿期组ARFI检测剪切波速度为(1.43±0.46)m/S,肝硬化失代偿期组(1.0±0.27)m/S,正常对照组的ARFI检测剪切波速度为(1.32±0.24)m/S,通过四组检验者的ARFI检测比较,比较差异均具有统计学意义(P<0.05)。而肝硬化代偿期组和正常对照组相比较,两组检验者的ARFI检测比较无统计学意义(P>0.05)。其中慢性肝病组患者的ARFI检测值为(2.47±0.57)m/S,比肝硬化失代偿期组的ARFI测值要高,两者的比较差异具有统计学意义(P<0.05)。而慢性肝病并发肝癌患者肿瘤边缘部位的ARFI测值为(3.65±0.87)m/S,和慢性肝病的背景进行比较,具有统计学意义(P<0.05)。结论通过研究表明,使用声脉冲辐射力成像技术可以有效反应出慢性肝病患者的弹性硬度,并且还可以间接的评价肝纤维化,具有较好的临床应用价值。
Objective To analyze the diagnostic value of acoustic pulse radiation imaging in the diagnosis of liver fibrosis in chronic liver disease. Methods Patients with liver disease from our hospital from June 2013 to October 2014 were randomly divided into two groups: chronic liver disease group (n = 60), decompensated liver cirrhosis group (n = 45) , Decompensated liver cirrhosis group (45 cases), and normal control group (50 cases). The four groups of patients were detected by ARFI, and the four groups of inspectors were evaluated the diameter of portal vein, Flow rate and ARFI do statistical analysis and comparison. Results The shear wave velocity of ARFI in chronic liver disease group was (2.34 ± 0.64) m / s, and the shear wave velocity of ARFI in decompensated liver cirrhosis group was (1.43 ± 0.46) m / s. The decompensated liver cirrhosis group (1.0 ± 0.27) m / s, ARFI shear wave velocity was (1.32 ± 0.24) m / S in the normal control group, and there was significant difference between the four groups ). However, compared with the normal control group, there was no significant difference in ARFI between the two groups (P> 0.05). The ARFI value of patients with chronic liver disease was (2.47 ± 0.57) m / S, which was higher than that of patients with decompensated liver cirrhosis. The difference between the two groups was statistically significant (P <0.05). The ARFI value of the edge of tumor in patients with chronic liver disease complicated with liver cancer was (3.65 ± 0.87) m / S, which was statistically significant compared with the background of chronic liver disease (P <0.05). Conclusions The study shows that the use of acoustic pulsed radiation imaging technology can effectively reflect the elastic stiffness of patients with chronic liver disease, and can also indirectly evaluate the liver fibrosis, and has good clinical value.
