目的:探讨彩色多普勒能量图(CDE)及频谱多普勒形态在鉴别乳腺良恶性肿块中的价值。方法:对经手术病理证实的53例乳腺癌及57例乳腺良性肿块的CDE及频谱多普勒形态特征进行回顾性分析。结果:①CDE检测结果:乳腺癌81.11%可探及穿入性动脉血流,内多呈血管网或血管树;良性肿块73.68%血流信号呈星点状或短线状,多位于周边。两组间比较差异有统计学意义(P<0.01)。②收缩期频谱多普勒形态特征:乳腺癌66.67%峰值前移,78.43%上升支及下降支陡直;良性肿块64.00%峰值居中,64.00%上升支及下降支斜平。舒张期频谱多普勒形态特征:乳腺癌86.27%舒张末期无血流或反向血流,良性肿块70.00%舒张末期同向血流。两组间比较差异有统计学意义(P<0.01)。结论:CDE及频谱多普勒形态在鉴别乳腺良恶性肿块中具有较高的诊断价值。
Objective: To investigate the value of color Doppler energy imaging (CDE) and spectral Doppler morphology in the differential diagnosis of benign and malignant breast tumors. Methods: The CDE and spectral Doppler features of 53 cases of breast cancer and 57 cases of benign breast lesions confirmed by surgery and pathology were retrospectively analyzed. Results: ① CDE test results: 81.11% of breast cancer can be detected and penetrating arterial blood flow, mostly vascular network or vascular tree; 73.68% benign mass signal was star-shaped or short-shaped, mostly located in the periphery. The difference between the two groups was statistically significant (P <0.01). (2) Doppler features of systolic frequency spectrum: 66.67% of breast cancer progressed to the peak, 78.43% of the rising and descending stents were steepest; 64.00% of the masses of the benign mass were peaked, 64.00% of the descending branches and descending diablo. Diastolic spectrum Doppler Morphological characteristics: Breast cancer 86.27% end-diastolic no blood flow or reverse blood flow, benign mass 70.00% end-diastolic with the same blood flow. The difference between the two groups was statistically significant (P <0.01). Conclusion: CDE and Doppler spectrum have high diagnostic value in differentiating benign and malignant breast tumors.
