目的观察全身伽玛刀配合化疗治疗局部晚期非小细胞肺癌(nonsmallcelllungcancer,NSCLC)的近期疗效及不良反应。方法对48例局部晚期NSCLC患者用全身伽玛刀放疗,同时配合全身化疗,每位病人至少完成2个周期化疗,末次化疗结束后4周以上评价疗效。结果完全缓解(CR)为14.6%,部分缓解(PR)77.1%,总有效率(CR+PR)91.7%;血液毒性主要为白细胞下降,总发生率100%,其中3、4度白细胞下降率为66.7%;放射性食管炎的发生率为8.3%,放射性肺炎为14.6%,均为1、2度。结论全身伽玛刀配合化疗治疗局部晚期NSCLC近期疗效满意,其毒副反应患者可以耐受,无治疗相关性死亡。
Objective To observe the short-term curative effect and adverse reactions of the whole body gamma knife combined with chemotherapy for the treatment of locally advanced nonsmall cell lung cancer (NSCLC). Methods 48 cases of locally advanced NSCLC patients with whole body gamma knife radiotherapy, combined with systemic chemotherapy, each patient completed at least 2 cycles of chemotherapy, the last chemotherapy after 4 weeks to evaluate the efficacy. Results The complete remission (CR) was 14.6%, partial remission (PR) 77.1% and total effective rate (CR + PR) 91.7%. The main hematological toxicity was leukopenia with a total incidence of 100% Was 66.7%; the incidence of radiation esophagitis was 8.3%, radiation pneumonitis was 14.6%, both 1,2 degrees. Conclusion The whole body gamma knife combined with chemotherapy in the treatment of locally advanced non-small cell lung cancer (NSCLC) has satisfactory short-term efficacy and patients with toxic side effects can tolerate without treatment-related death.
