目的检测乙型肝炎相关性原发性肝细胞癌患者(hepatocellular carcinoma,HCC)HBV tDNA、HBVcccDNA,并探讨其与乙型肝炎病毒表面抗原(HBsAg)定量的相互关系。方法应用实时荧光PCR方法对37例HCC患者癌组织和癌旁组织HBV tDNA、HBV cccDNA及血清HBV tDNA进行定量检测;采用雅培化学发光法进行HBsAg定量检测。结果癌组织HBV tDNA、HBV cccDNA与HBsAg定量无相关性(r=0.146,P=0.388;r=0.151,P=0.371);癌旁组织HBV tDNA、HBV cccDNA与HBsAg定量无相关性(r=0.323,P=0.051;r=0.186,P=0.271)。癌组织HBV tDNA、HBV cccDNA及HBV cccDNA占HBV tDNA比例与癌旁组织比较差异均无统计学意义(6.61 lg拷贝/μgvs6.26 lg拷贝/μg,P=0.067;0.96 lg拷贝/μgvs1.33lg拷贝/μg,P=0.438;14.23%vs19.65%,P=0.501);HBV复制指标间相关性弱,仅癌组织HBV cccDNA与外周血HBV tDNA存在相关性(r=0.382,P=0.020),癌旁组织HBV tDNA与HBV cccDNA存在相关性(r=0.327,P=0.048),癌与癌旁组织HBV tDNA存在相关性(r=0.665,P<0.001)。结论 HCC患者病毒复制水平减低,癌与癌旁组织病毒复制水平无明显差异,且与血清HBsAg水平亦无明显相关性。
Objective To detect HBV tDNA and HBVcccDNA in patients with hepatocellular carcinoma (HCC) related to hepatitis B and to explore their relationship with the quantitative of hepatitis B virus surface antigen (HBsAg). Methods The real-time fluorescent PCR method was used to detect the HBV tDNA, HBV cccDNA and serum HBV tDNA in 37 cases of HCC patients and the HBsAg quantitative detection by Abbott chemiluminescence method. Results There was no correlation between HBV tDNA, HBV cccDNA and HBsAg in cancerous tissues (r = 0.146, P = 0.388; r = 0.151, P = 0.371) , P = 0.051; r = 0.186, P = 0.271). There was no significant difference in the proportion of HBV tDNA, HBV cccDNA and HBV cccDNA between the HBV tDNA and the adjacent non-cancerous tissues (6.61 lg copies / μgvs6.26 lg copies / μg, P = 0.067; 0.96 lg copies / μgvs 1.33 lg copies / g, P = 0.438; 14.23% vs 19.65%, P = 0.501). The correlation between HBV replication index was weak. Only the correlation between HBV cccDNA and HBV tDNA in peripheral blood (r = 0.382, There was a correlation between HBV tDNA and HBV cccDNA (r = 0.327, P = 0.048) in paracancerous tissues and HBV tDNA in adjacent tissues (r = 0.665, P <0.001). Conclusions The level of virus replication in HCC patients is lower than that in non-HCC patients, and there is no significant difference between HCC patients and serum HBsAg levels.
