Objective To investigate the changes of endothelin (ET) in plasma and cerebrospinal fluid (CSF) during central nervous system infection (CNSI). Methods The levels of ET-1 in plasma and CSF of 66 patients with CNSI were determined by radioimmunoassay. Results The level of CSF in CNSI patients was higher than that in control group, and there was significant difference between control group and virus brain and tuberculin group (P <0.001, 0.05 respectively), but no significant difference (P> 0.05). There was no significant correlation between plasma levels and CNSI (P> 0.05). The levels of CSF and plasma ET-1 in patients with CNSI complicated with severe brain dysfunction were significantly higher than those with mild to moderate brain dysfunction (P <0.001) or those without brain dysfunction (P <0.001). The mild to moderate brain dysfunction was significantly higher than those without brain dysfunction (P <0.001). Conclusions ET-1 level in CSF and plasma is not related to CNSI disease, but is related to the degree of brain injury. Increased levels of ET-1 may be an important index of brain injury. ET-1 may be an important factor in brain injury
