目的研究纳米晶羟基磷灰石和胶原的复合材料(NHAC)结合自体骨膜一期修复儿童颅骨缺损。方法实验组17例患儿术中去骨瓣减压后,颅骨缺损区接受NHAC移植治疗。对照组为11例同期行去骨瓣减压而未植入NHAC的患儿。定期CT检查并比较两组患儿颅骨缺损的修复情况。结果实验组除1例术后接受放疗颅骨再生失败,其他骨缺损区达到基本愈合;对照组仅3例达到基本愈合,实验组疗效显著优于对照组(P<0.01)。结论 NHAC一期修补颅骨既可达到减压目的,又可避免再次颅骨修补,是治疗儿童颅骨缺损的理想方法。
Objective To study the effect of nano-hydroxyapatite (HA) and collagen composite (NHAC) combined with autologous periosteum on the repair of skull defects in children. Methods 17 cases of experimental group children with intraoperative decompressive craniectomy received NHAC transplantation. The control group consisted of 11 children who had undergone decompression of the cortical bone without NHAC. Regular CT examination and comparison of two groups of children with skull defect repair. Results In the experimental group, the healing of the skull was failed in one case after radiotherapy and the other bone defects were basically healed. Only three cases in the control group basically healed. The therapeutic effect in the experimental group was significantly better than that in the control group (P <0.01). Conclusions The first stage of NHAC repair skull can achieve decompression purposes, but also to avoid the skull repair again, is an ideal method for the treatment of skull defects in children.
