目的研究健康成年人空腹胃肠动力及血浆胃动素浓度的变化,以探讨2者的关系。方法采用灌注式测压方法,检测15例健康志愿者空腹胃肠动力,并根据压力波形特点分期抽血采用放免法检测血浆胃动素浓度。结果所有志愿者空腹胃肠运动都表现为反复发生的运动复合波(migrating motor complex,MMC),MMC周期平均持续106±32 min,根据压力波形特点MMC可分为3期。Ⅰ期平均持续38.9±22.4 min;Ⅱ期平均持续62.3±20.1 min;胃窦Ⅲ期持续2.8±1.2 min,十二指肠Ⅲ期持续4.5±1.8 min。MMC各期的血浆胃动素浓度不相等(P<0.05),Ⅱ期和Ⅲ期的胃动素浓度均较Ⅰ期高(P<0.05),Ⅱ期和Ⅲ期血浆胃动素浓度相比较无统计学差异(P>0.05)。结论健康成年人血浆胃动素浓度随MMC周期波动,胃动素可能参与MMC的发生。
Objective To study the changes of fasting gastrointestinal motility and plasma motilin in healthy adults to explore the relationship between the two. Methods The fasting gastrointestinal motility of 15 healthy volunteers was detected by perfusion manometry. Plasma motilin was determined by radioimmunoassay according to the characteristics of pressure waveform. Results The fasting gastrointestinal motility of all volunteers showed recurrent migrating motor complex (MMC). The duration of MMC continued for an average of 106 ± 32 min. According to the characteristics of pressure waveform, MMC could be divided into three phases. The average duration of the first phase was 38.9 ± 22.4 min, the average duration of the second phase was 62.3 ± 20.1 min, the third phase of the gastric phase continued for 2.8 ± 1.2 min, and the third phase of the duodenum continued for 4.5 ± 1.8 min. Plasma motilin concentrations were not equal in all stages of MMC (P <0.05), motilin levels in stage II and III were higher than those in stage I (P <0.05), plasma concentrations of motilin in stage II and III No statistical difference (P> 0.05). Conclusion Plasma concentrations of motilin in healthy adults fluctuate with the MMC cycle. Motilin may be involved in the development of MMC.
