目的:评价长期中等强度运动训练对慢性心力衰竭(CHF)病人血浆心型脂肪酸结合蛋白(H-FABP)的影响。方法:采用药物标准治疗的60例CHF病人被随机分为两组:运动训练组(30例),常规治疗组(30例),另设正常对照组(30例)。于治疗前、后抽血检测血清脑钠肽(BNP)及H-FABP水平,并进行比较分析。结果:CHF患者运动训练组及常规治疗组治疗前血清BNP[(357±32)mg/ml,(343±14)mg/ml:(36±4)mg/ml]、H-FABP[(15.5±3.1)μg/ml,(13.0±5.4)μg/ml:(4.1±2.3)μg/ml]水平均显著高于正常对照组(P均<0.05);与治疗前比较,运动组运动训练6个月后BNP[(357±32)mg/ml:(104±41)mg/ml],H-FABP[(15.5±3.1)μg/ml:(6.1±4)μg/ml]明显下降(P<0.01),常规治疗组治疗后BNP[(343±14)mg/ml:(165±20)mg/ml],H-FABP[(13.0±5.4)μg/ml:(8.2±5.1)μg/ml]也有明显下降(P<0.05),但运动组疗效优于常规治疗组(P<0.05)。血清H-FABP、BNP水平随心衰加重而明显升高(P<0.05),H-FABP与BNP呈正相关(r=0.39,P<0.05)结论:长期适当的运动训练,可使慢性心力衰竭病人血浆心型脂肪酸结合蛋白的水平显著下降。
Objective: To evaluate the effect of long-term moderate-intensity exercise training on plasma cardio-fatty acid binding protein (H-FABP) in patients with chronic heart failure (CHF). Methods: Sixty CHF patients treated with drug standard were randomly divided into two groups: exercise training group (30 cases), conventional treatment group (30 cases) and normal control group (30 cases). Before and after treatment, blood samples were taken for determination of serum brain natriuretic peptide (BNP) and H-FABP levels, and comparative analysis was conducted. Results: The serum levels of BNP [(357 ± 32) mg / ml and (343 ± 14) mg / ml: 36 ± 4 mg / ml] and H-FABP [(15.5 ± 3.1) μg / ml, (13.0 ± 5.4) μg / ml: (4.1 ± 2.3) μg / ml] were significantly higher than those in the normal control group (all P <0.05) The levels of BNP [(357 ± 32) mg / ml: (104 ± 41) mg / ml] and H-FABP [(15.5 ± 3.1) μg / ml: 0.01). The BNP levels in the conventional treatment group were significantly higher than those in the control group (P <0.05) ml] (P <0.05), but the exercise group was better than the conventional treatment group (P <0.05). The levels of serum H-FABP and BNP were significantly increased with the worsening of heart failure (P <0.05), and the positive correlation between H-FABP and BNP (r = 0.39, P <0.05) .Conclusion: Long-term proper exercise training can make chronic heart failure Patient plasma cardiogenic fatty acid-binding protein levels decreased significantly.
