目的:探讨不同出生体重早产新生儿早期营养对出生后两周内体重变化的影响。方法:采用定群研究和前瞻性观察方法对2007年10月~2008年10月入住某省立儿童医院新生儿科的早产儿能量、蛋白质摄入与体重增长情况进行评估。按照出生体重不同,将符合条件的177例研究对象分为体重<2 000 g组和体重≥2 000 g组,比较两组能量-蛋白质摄入对体重增长的影响。结果:体重<2 000 g组和体重≥2 000 g组出生后第1周内平均每天每1 000 g总能量、蛋白质的摄入量差异没有统计学意义,第2周有统计学意义(t分别为-2.571、-5.390,均P<0.05)。体重<2 000 g组恢复出生体重日龄和体重下降最大幅度明显高于体重≥2 000 g组(t分别为1.367、2.426,均P<0.05),经绘制曲线图发现,2周内,体重≥2 000 g组体重增长值均高于体重<2 000 g组。结论:两组在住院期间2周内能量和蛋白质摄入普遍不足,均未能达到理论要求。早产儿,尤其是出生体重较低者应该尽早进行合理的营养,补足能量-蛋白质需求。
Objective: To investigate the effects of early nutrition of preterm newborns with different birth weight on body weight changes within two weeks after birth. Methods: The population of energy, protein intake and weight gain in preterm infants admitted to a Provincial Children’s Hospital from October 2007 to October 2008 were evaluated by population study and prospective observation. According to the different birth weight, 177 eligible subjects were divided into two groups with body weight <2000 g and body weight ≥ 2000 g. The effects of energy-protein intake on body weight gain were compared between the two groups. Results: The average daily energy per 1 000 g of body weight <2000 g group and body weight ≥ 2000 g group during the first week after birth did not reach statistical significance (P <0.05), and there was no significant difference between the two groups Respectively -2.571, -5.390, all P <0.05). Premature infants, especially those born at lower birth weight, should be sensitized as early as possible to meet energy-protein needs.
