患者陶××,女性,31岁,住院号36538,关节疼痛十余年,心慌气急5—6年,近年来加剧而入院。检查:心率100次/分,律不齐,心尖部可闻及中度隆隆样D、M,胸骨左缘3—4肋间闻及开瓣音。X光所见:左房右室增大为主.E.K.G.:房颤,心率110次/分,伴室内差传,右室肥厚。“B”型超声。1.右室肥大负荷加重;2.左房扩大; 3.二尖瓣肥厚.活动度小;4.心律不齐,房颤、诊断:风湿性心脏病二尖瓣狭窄,用地戈辛0.25mgl/日,后将剂量减半,术前两天停药。于83年3月19日在静脉全麻下行二尖瓣扩张分离术,术前半小时肌注杜冷丁40mg及东莨菪硷0.3mg、8 30/Am入手术室。术前血压100/70mmHg。心率100次/分,心电监
