目的探讨胰腺脂肪浸润对营养性肥胖大鼠的胰岛功能及形态影响。方法刚断乳SD雄性大鼠70只,随机分为普通饲料组(N组)和高脂饲料组(F组),18周后成功构建营养性肥胖大鼠模型34只,将其按体重分为轻度肥胖组(A组,n=11)、中度肥胖组(B组,n=11)、重度肥胖组(C组,n=12)。测空腹胰岛素(FINS)、游离脂肪酸(FFA)、白介素-6(IL-6)、肿瘤坏死因子α(TNFα)、胰腺组织甘油三酯(TG)和胰腺组织病理改变等指标。计算胰岛素抵抗指数(HOMA-IR)、胰岛素β细胞功能指数(HOMA-β)、早期胰岛素分泌功能指数(ΔI30/ΔG30)及胰岛素曲线下面积(0~120 min)。结果 C组大鼠胰腺TG含量比N组明显增高[(324.52±55.82)μmol/L vs(269.57±50.45)μmol/L,P<0.05)]。与N组相比,B组HOMA-IR显著升高[(2.27±0.31)vs(1.91±0.26),P<0.01];B、C组的ΔI30/ΔG30明显降低[(8.90±2.23)vs(12.56±3.38),P<0.05;(7.41±2.00)vs(12.56±3.38),P<0.01]。结论轻中度胰腺脂肪浸润可造成胰岛素早期分泌时相延迟,出现胰岛素抵抗;而重度胰腺脂肪浸润可致胰岛β细胞损伤。
Objective To investigate the effects of pancreatic fat infiltration on pancreatic islet function and morphology in rats with nutritional obesity. Methods Seventy-eight SD male SD rats were randomly divided into normal diet group (N group) and high fat diet group (F group). After 18 weeks, 34 SD rats were successfully established, They were mild obesity (group A, n = 11), moderate obesity (group B, n = 11) and severe obesity (group C, n = 12). The levels of fasting insulin (FINS), free fatty acids (FFA), interleukin-6 (IL-6), tumor necrosis factor alpha (TNFα), pancreatic tissue triglyceride (TG) The HOMA-IR, HOMA-β, early insulin secretion index (ΔI30 / ΔG30) and the area under the insulin curve (0-120 min) were calculated. Results The content of TG in pancreas in group C was significantly higher than that in group N [(324.52 ± 55.82) μmol / L vs (269.57 ± 50.45) μmol / L, P <0.05). Compared with group N, HOMA-IR of group B was significantly higher than that of group B [(2.27 ± 0.31) vs (1.91 ± 0.26), P <0.01]; ΔI30 / ΔG30 was significantly decreased in group B and C 12.56 ± 3.38), P <0.05; (7.41 ± 2.00) vs (12.56 ± 3.38), P <0.01]. Conclusions Mild and moderate pancreatic fat infiltration may result in the delay of early insulin secretion and insulin resistance. However, severe pancreatic fat infiltration may cause islet β cell injury.
