目的 抽查处方的合格率及了解临床不合理用药情况 ,提高合理用药水平。方法 从本院随机抽取 10 0 0张住院及门诊处方 ,发现书写不规范 35例 ,不合理用药处方 10 3例 ,对此进行分析、讨论。结果 处方书写不规范、药物使用时的剂量、用法有误 ,毒副作用叠加 ,配伍不合理 ,滥用抗生素 ,重复用药等现象居多。结论 处方书写不规范及不合用药应该引起临床医师的高度重视 ,药师参与督导用药 ,以减少药不良反应及药源性疾病的发生 ,确实提高临床合理用药水平。
Objective To check the passing rate of prescriptions and to understand the situation of clinical unreasonable drug use and improve the reasonable level of medication. Methods A total of 100 inpatient and outpatient prescriptions were randomly selected from our hospital, and 35 cases were not standardized and 103 cases were irrational, which were analyzed and discussed. The results of prescription writing is not standardized, the dosage of drugs used, usage is wrong, the superposition of toxic and side effects, incompatibility, abuse of antibiotics, repeated medication and other phenomena. Conclusion Nonstandard prescriptions and incompatibility drugs should be taken seriously by clinicians. Pharmacists take part in supervising medication to reduce adverse drug reactions and drug-induced diseases, and indeed improve the clinical reasonable medication level.
