本研究以36例PCOS妇女和20例正常月经周期妇女为研究对象。通过测定激素基础水平,观察在糖耐量试验中胰岛素对糖负荷的反应和睾酮的变化以及分析体脂与胰岛素的关系。发现PCOS妇女有较高的空腹血糖水平、空腹血清胰岛素与空腹血清睾酮呈显著正相关(r=0.65,P<0.01)。在糖负荷期PCOS组血清胰岛素释放显著高于对照组(P<0.01),同时,总睾酮与总胰岛素呈正相关(r=0.69,P<0.01)。PCOS肥胖者总胰岛素对糖负荷的反应最大,对照非肥胖者反应最小(F=3.02,P<0.01)。PCOS妇女体脂百分率与空腹血清胰岛素呈正相关(r=0.523,P<0.05)。由此提示(1)PCOS妇女内源性胰岛素释放是PCOS雄激素增高的原因之一;(2)PCOS妇女体脂过多可能在PCOS的高胰岛素血症和胰岛素抵抗中起有部分作用。
In this study, 36 cases of PCOS women and 20 cases of normal menstrual cycle women as the research object. By measuring the basal level of hormones, the insulin response to glucose load and testosterone changes in glucose tolerance test were observed, and the relationship between body fat and insulin was analyzed. Found that PCOS women have higher fasting blood glucose levels, fasting serum insulin and fasting serum testosterone was significantly correlated (r = 0.65, P <0.01). Serum insulin release in PCOS group was significantly higher than that in control group (P <0.01). Meanwhile, total testosterone was positively correlated with total insulin (r = 0.69, P <0.01). Total insulin in PCOS obese responds most to sugar load and responds to non-obese responders (F = 3.02, P <0.01). Body fat percentage in PCOS women was positively correlated with fasting serum insulin (r = 0.523, P <0.05). This suggests that (1) PCOS women with endogenous insulin release is one of the causes of PCOS androgen escalation; (2) PCOS women with excessive body fat may have some role in hyperinsulinemia and insulin resistance in PCOS.
