近年来,痛风的发病率逐年增高,为探讨本病的临床特点,以期提高确诊率,减少发作频率,现将我院近年收治的临床资料完整的42例原发性痛风作一临床分析.1 临床资料1.1 一般情况 42例均为男性.确诊时年龄31～74岁.平均53.7岁;职业:脑力劳动者32例,体力劳动者10例;病程7天～30年,平均5.72年.诊断根据1985年美国Holmes提出的标准(陈慎仁,陈俊辉.痛风的诊断和治疗策略.新医学,1994,25(8):437),具备其中的一条并排除了继发性痛风.1.2 临床表现 主要为急性痛风性关节炎,发生在第(?)趾关节17例,踝关节11例,手指关节及膝关节各8例,其他足趾关节7例,腕关节及足背各3例,小腿部2例,肘关节1例.其中多个关节同时受累12例,有痛风石8例.起病急剧,受累关节或局部表现为红、肿、热、痛,疼痛以夜间为甚,持续数天,甚至2～3周后局部皮肤脱皮发痒,皮色呈紫红色,全身症状多较轻微.伴畏寒、发热7例,体温最高达38.8℃.

Among them, a number of joints involved in 12 cases at the same time, there are 8 cases of tophi. Rapid onset, affected joints or local performance of redness, swelling, heat, pain, pain even more at night for several days, Even 2 to 3 weeks after the local skin peeling itchy, skin color was purple, systemic symptoms are more minor with chills, 7 cases of fever, body temperature up to 38.8 ℃.
