目的探讨乳腺良恶性病灶MRI动态增强半定量参数与血管内皮生长因子(VEGF)的相关性。方法44例乳腺病患者经病理证实分为良恶性2组,术前行MRI动态增强后计算时间-信号曲线半定量参数,并对所有病灶进行CD34和VEGF免疫组化染色的阳性表达进行评分,评价半定量参数与其相关性。结果乳腺癌24例,乳腺良性病灶20例。恶性组中最大排泄率(Ewash)、最大排泄速率(Vwash)及最大相对强化斜率(Eslop)与VEGF评分有显著性正相关(r=0.561,0.522,0.573,P<0.01),而达峰时间(Tmax)与VEGF评分呈一定的负相关(r=-0.418,P<0.05);良性组的强化参数与VEGF评分无明显相关性(P>0.01)。恶性组中VEGF阳性组的微血管密度(MVD)显著性高于阴性组,2组有显著性差异的参数是最大强化速率(Vmax)(P<0.01),而良性组中VEGF阳性组与阴性组无明显差异(P>0.01)。结论恶性肿瘤MRI T1动态增强半定量参数与血管内皮生长因子具有一定的相关性。
Objective To investigate the relationship between dynamic semi-quantitative MRI parameters and vascular endothelial growth factor (VEGF) in benign and malignant breast lesions. Methods Forty-four patients with breast cancer were divided into benign and malignant groups according to pathology. Semi-quantitative parameters of time-signal curve were calculated by dynamic MRI before operation. All the lesions were scored by immunohistochemical staining of CD34 and VEGF. Evaluation of semi-quantitative parameters and their relevance. Results of 24 cases of breast cancer, breast benign lesions in 20 cases. There was a significant positive correlation between Ewash, Vwash and Eslop in malignant group and VEGF score (r = 0.561, 0.522, 0.573, P <0.01), but peak time There was a negative correlation between Tmax and VEGF score (r = -0.418, P <0.05). There was no significant correlation between Tmax and VEGF score in benign group (P> 0.01). In the malignant group, the microvessel density (MVD) of the VEGF-positive group was significantly higher than that of the negative group (P <0.01), while the difference between the two groups was Vmax (P <0.01) No significant difference (P> 0.01). Conclusion The semi-quantitative parameters of MRI T1 dynamic enhancement of malignant tumors have a certain correlation with VEGF.
