目的探讨胸腔镜下Hem-o-lok结扎锁治疗食管癌术后乳糜胸的效果。方法选择食管癌术后并发大量乳糜胸患者5例,经右胸腔镜下使用Hem-o-lok结扎锁行胸导管膈上大块组织结扎。结果 5例患者手术均获成功,无中转开胸。手术时间63-95min,术中出血量55-110ml。术后首日胸腔引流量200-385ml,引流时间4-8d。出院前复查胸片双肺膨胀良好,胸腔无明显积液。半年后随访无复发。结论胸腔镜下Hem-o-lok结扎锁治疗食管癌术后乳糜胸安全、可靠、便捷。
Objective To investigate the effect of thoracoscopic Hem-o-lok ligation in the treatment of postoperative chylothorax in patients with esophageal cancer. Methods A total of 5 patients with large chylothorax after esophageal cancer operation were selected and ligated through the thoracoscopy using Hem-o-lok ligation. Results All the 5 cases were operated successfully without thoracotomy. Surgery time 63-95min, intraoperative blood loss 55-110ml. The first day postoperative chest drainage 200-385ml, drainage time 4-8d. Chest radiography before discharge chest lung expansion, no significant pleural effusion. Six months after follow-up no recurrence. Conclusions Thoracoscopic Hem-o-lok ligation is safe, reliable and convenient for the treatment of postoperative chylothorax in patients with esophageal cancer.
