目的探讨24 h动态收缩压(SBP)与老年人群臂踝脉搏波传导速度(ba PWV)的关联性。方法 2006~2007年第1次对开滦集团公司在职及离退休职工进行健康体检,此后分别于2008~2009、2010~2011年进行第2、3次健康体检,并于第3次体检时整群随机抽取年龄≥60岁的开滦集团离退休员工2814例进行动态血压监测、ba PWV测定等检查。符合入选标准的为2464例,资料完整者1004例。采用多因素线性回归分别分析24h SBP、日间SBP、夜间SBP与ba PWV的关联性。结果 (1)按照24h SBP、日间SBP、夜间SBP三分位数分别将研究对象分为3组,随着24h SBP、日间SBP、夜间SBP升高,ba PWV均增加(均P<0.001)。(2)多因素线性回归分析显示:校正混杂因素后,24h SBP、日间SBP、夜间SBP每增加1个标准差,ba PWV分别增加81.15、73.18、81.30 cm/s;此外,诊室SBP也与ba PWV呈线性正相关,且回归系数大于动态SBP。结论 24h SBP、日间SBP、夜间SBP与ba PWV呈正相关,但关联性弱于诊室SBP。
Objective To investigate the association between dynamic systolic pressure (SBP) at 24 h and brachial-ankle pulse wave velocity (ba PWV) in the elderly. Methods The first physical examination of serving and retired workers of Kailuan Group from 2006 to 2007 was conducted, and the second and third physical examinations were carried out from 2008 to 2009 and from 2010 to 2011, respectively. During the third physical examination, A total of 2,814 retired staff from Kailuan Group, aged> 60 years, were randomly selected for ambulatory blood pressure monitoring and ba PWV test. Meet the inclusion criteria for the 2464 cases, 1004 cases of complete data. Multivariate linear regression was used to analyze the association of 24h SBP, daytime SBP, night SBP with ba PWV. Results (1) Subjects were divided into three groups according to 24h SBP, daytime SBP and nocturnal SBP tertiles. With 24h SBP, daytime SBP and nocturnal SBP, ba PWV were increased (all P <0.001 ). (2) Multivariate linear regression analysis showed that ba PWV increased by 81.15, 73.18 and 81.30 cm / s for each additional SBP, daytime SBP and nighttime SBP after adjusting for confounding factors. In addition, SBP was also associated with ba PWV is linearly correlated, and the regression coefficient is greater than the dynamic SBP. Conclusions 24h SBP, daytime SBP, nighttime SBP were positively correlated with ba PWV, but the correlation was weaker than that of SBP.
