目的分析乳腺癌胸壁电子线放射治疗(EBCWI)的摆位固定方法,改进现有固定体模,提高摆位精度。方法对照组采用常规的软枕固定体位,试验组采用改进后的负压真空垫固定,两组各收集15例EBCWI摆位数据,分析摆位精度。结果 30例患者放射治疗过程中X、Y方向均有不同程度的移动,试验组X、Y方向偏移为(3.48±0.91)mm/(2.13±0.61)mm,对照组X、Y方向偏移为(6.15±1.14)mm/(4.26±1.25)mm,差异有统计学意义(P<0.01);对照组射野中心SSD距离>100 cm的16次(21.3%)高于试验组7次(9.3%),两组差异存在明显统计学意义(P<0.05);射野水平度试验组优于对照组,差异有统计学意义(P<0.01)。结论 30°楔形负压真空垫固定组在摆位的固定性、重复性优于普通靠垫固定组,而且操作简单易行,误差小,能更好地保证放疗质量。
Objective To analyze the setting and fixing method of electronic chest radiography (EBCWI) in breast cancer, to improve the existing fixed phantom and improve the positioning precision. Methods The control group received conventional soft pillow fixation, and the experimental group was fixed with the improved negative pressure vacuum pad. The data of 15 cases of EBCWI were collected and the accuracy of setting was analyzed. Results The X and Y directions of the 30 patients were all moved to different degrees during the radiotherapy. The X and Y directions were (3.48 ± 0.91) mm / (2.13 ± 0.61) mm in the experimental group, and the X and Y directions were shifted in the control group (6.15 ± 1.14) mm / (4.26 ± 1.25) mm, the difference was statistically significant (P <0.01). In the control group, the incidence of SSD was> 100 cm (16.3% 9.3%). The difference between the two groups was statistically significant (P <0.05). The level of field test was superior to that of the control group (P <0.01). Conclusion The 30 ° wedge negative pressure vacuum pad fixation group has better fixation and repeatability than ordinary cushion fixation group, and the operation is simple and easy, the error is small, and the radiotherapy quality can be better guaranteed.
