目的:探索药物入壶后输出液药物浓度峰值变化规律并分析其影响因素,为临床规范应用这一给药方式提供理论依据。方法:用紫外可见光光度仪连续监测龙胆紫饱和溶液入壶后输出液体的吸光度,将测得的吸光度转换成药物浓度,获得输出液药物浓度峰值。分别设定输液速度为90,180,360,720 ml·h-1;输液器滴壶内液体量分别设定为2,3,4 ml;入壶溶液分别为1,2 ml。对实验结果进行统计学分析。结果:输液速度(x1)、滴壶内液体量(x2)、入壶药量(x3)对输出液药物浓度峰值(y)均有影响,差异具有统计学意义(P<0.05)。对于3个影响因素的多元线性回归分析得到^y=7.283+0.003 x1-1.904x2+8.820 x3(R2=0.947,F=547.080),三者影响由大到小依次为:入壶药量、壶内液量、输液速度。入壶药量越大,输出液药物浓度峰值越高,壶内液量越多输出液药物浓度峰值越低,输液速度越快输出液药物浓度峰值越高。结论:入壶给药时输出液药物浓度受多种因素影响,与静脉注射药物浓度变化规律不同,一些药物不能采用入壶方式代替静脉注射。
OBJECTIVE: To explore the change law of peak value of drug concentration after drug into the jug and to analyze its influencing factors to provide a theoretical basis for clinical application of this mode of administration. Methods: UV absorbance spectrophotometer was used to continuously monitor the absorbance of the output liquid of gentian violet saturated solution into the kettle. The measured absorbance was converted into the drug concentration to obtain the peak value of the output drug concentration. Respectively set the infusion rate of 90,180,360,720 ml · h-1; Infusion drip pot liquid volume were set to 2,3,4 ml; into the pot solution were 1,2 ml. The experimental results were statistically analyzed. Results: The speed of infusion (x1), the volume of liquid in drip pot (x2) and the amount of drug in pot (x3) had significant effects on the peak value of drug concentration (y) in the effusion fluid, with statistical significance (P <0.05). The multiple linear regression analysis of the three influencing factors showed that: y = 7.283 + 0.003 x1-1.904x2 + 8.820 x3 (R2 = 0.947, F = 547.080) Fluid volume, infusion rate. The greater the amount of drug into the pot, the higher the peak value of the drug concentration of the output liquid, the more the amount of fluid in the pot the lower the peak value of the drug concentration of the output liquid, the faster the infusion rate the higher the peak drug concentration of the output liquid. Conclusion: The drug concentration in the effluent is affected by many factors when administered into the jug. Unlike the changes in the concentration of intravenous drug, some drugs can not be used instead of the intravenous injection.
