目的探讨宫颈病变组织中P16、P27和Ki67蛋白的表达情况及临床意义。方法采用免疫组化S-P法检测30例宫颈上皮内瘤样病变(CIN)、18例宫颈鳞癌组织和12例宫颈正常鳞状上皮组织中P16、P27和Ki67蛋白的表达,分析宫颈病变形成过程中P16、P27和Ki67蛋白表达的变化以及临床病理特征的关系。结果P16和Ki67在正常宫颈、CIN和宫颈鳞癌组织中的表达率分别为0(0/12)、70%(21/30)、100%(18/18)和30%(4/12)、90%(27/30)、100%(18/18),二者在CIN和宫颈鳞癌中阳性表达率明显高于正常宫颈组织的表达,差异有显著性(P<0.05);P27蛋白在正常宫颈、CIN和宫颈鳞癌组织中的表达率分别为82.22%(10/12)、53.33%(16/30)和27.78%(5/18),其在CIN和宫颈鳞癌中阳性表达率明显低于正常宫颈组织的表达,差异有显著性(P<0.05)。在CIN和宫颈鳞癌组织中,P16和Ki67表达呈正相关(P<0.05);P27和P16、Ki67表达呈负相关(P<0.05)。结论P16、P27和Ki67参与了CIN、宫颈癌的发生。P16、Ki67和P27联合检测可作为早期诊断宫颈上皮内瘤变及宫颈癌的标记物,可提高宫颈癌的早期诊断率。
Objective To investigate the expression of P16, P27 and Ki67 in cervical lesions and its clinical significance. Methods Immunohistochemical SP method was used to detect the expression of P16, P27 and Ki67 in 30 cases of cervical intraepithelial neoplasia (CIN), 18 cases of cervical squamous cell carcinoma and 12 cases of normal cervical squamous epithelium, and to analyze the formation of cervical lesions P16, P27 and Ki67 protein expression changes and clinicopathological features. Results The positive rates of P16 and Ki67 in normal cervix, CIN and cervical squamous cell carcinoma were 0 (0/12), 70% (21/30), 100% (18/18) and 30% (4/12) (P <0.05). The positive rate of P27 protein in cervical squamous cell carcinoma was significantly higher than that in normal cervical tissues (P <0.05) The positive rates of CIN and cervical squamous cell carcinoma were 82.22% (10/12), 53.33% (16/30) and 27.78% (5/18) respectively in cervical intraepithelial neoplasia, cervical squamous cell carcinoma and cervical intraepithelial neoplasia The rate of cervical cancer was significantly lower than that of normal cervical tissue (P <0.05). There was a positive correlation between P16 and Ki67 expression in CIN and cervical squamous cell carcinoma (P <0.05), but negative correlation between P27 and P16 and Ki67 (P <0.05). Conclusion P16, P27 and Ki67 are involved in CIN and cervical cancer. P16, Ki67 and P27 combined detection can be used as early diagnosis of cervical intraepithelial neoplasia and cervical cancer markers, can improve the early diagnosis of cervical cancer.
