目的:探讨四腔心观(FCV),左、右心室流出道观(VOTV)及三血管-气管观(3VT)四个超声基本切面在胎儿心脏病产前超声初筛中的临床意义。方法:应用彩色多普勒超声诊断仪,运用此四个标准切面对孕20~28周胎儿进行检查,共计检查例数1569例,与产后超声追踪对照和尸体解剖证实。结果:四腔心观左右心室流出道观与三血管观的超声诊断敏感性为90%(1412/1569),96%(1506/1569)的胎儿心脏缺陷尤其是严重畸形被发现。结论:初筛中只要显示好心脏四个基本切面可明显提高胎儿心脏病的诊断率,大大地缩短了检查时间,可以作为初筛胎儿心脏病的首选方法。
Objective: To investigate the clinical significance of prenatal ultrasound screening of fetal heart disease by four ultrasonographic views of four chambers (FCV), left and right ventricular outflow tract (VOTV) and trivascular - tracheal observation (3VT). Methods: The color Doppler ultrasound diagnostic apparatus, the use of these four standard sections of pregnant 20-28 weeks fetus examination, a total of 1569 cases of examination, and postnatal ultrasound tracking control and autopsy confirmed. Results: The sensitivity of four-chamber view of left ventricle and left ventricle outflow tract was 90% (1412/1569), while 96% (1506/1569) of fetal heart defects, especially severe deformity, were found. CONCLUSION: As long as the four basic sections of the heart are displayed in the preliminary screening, the diagnosis rate of fetal heart diseases can be obviously improved, the examination time can be greatly shortened, and the method can be used as the first choice for screening fetal heart diseases.
