某些拟β-肾上腺素能药物—如异丙肾上腺素、多巴胺、羟甲叔丁肾上腺素(Salbutamol)—干扰糖尿病患者的糖耐量,引起高血糖,某些病例可引起酮症酸中毒。作者报告一例用结构与多巴胺相似的人工合成强心剂多巴酚丁胺(dobutamine)引起糖尿病酮症酸中毒时胰岛素需要量的明显增加。 病例报告: 一、45岁女性胰岛素依赖性糖尿病患者处于严重的酮症酸中毒,昏迷、脱水,酮症,心率100次/分,血压90/40mmHg,无感染的临床证据。初步检查发现;血糖1,441mg%,重炭酸盐2mEq/L,氢离子18ng%,血红蛋白10.2g%,白细胞18,500/mm~3,胸部X线检查正常。治疗包括静脉输注5单位中性
Some proposed β-adrenergic drugs - such as isoproterenol, dopamine, and Salbutamol - interfere with impaired glucose tolerance in patients with diabetes, causing hyperglycemia and in some cases ketoacidosis. The authors report a significant increase in insulin requirements for diabetic ketoacidosis caused by dobutamine, a synthetic tympanogram similar in structure to dopamine. Case Reports: A 45-year-old woman with insulin-dependent diabetes is in severe ketoacidosis, coma, dehydration, ketosis, heart rate 100 beats / min, blood pressure 90 / 40mmHg, no clinical evidence of infection. Preliminary examination found; 1,441 mg of blood glucose, bicarbonate 2mEq / L, hydrogen ions 18ng%, hemoglobin 10.2g%, leukocytes 18,500 / mm ~ 3, chest X-ray examination was normal.
