目的:了解丽水市结核分枝杆菌耐EMB分离株embB基因突变情况。方法:采用PCR-DNA直接测序法对47株结核分枝杆菌临床分离株embB基因与耐EMB关系密切的突变位点进行分析。结果:药敏试验共获得EMB耐药结核菌7株,总耐药率为14.89%,获得性耐药率为16.67%;embB基因第306位点未见碱基的插入或缺失,共中有5株存在点突变,突变率为10.64%,均为EMB耐药菌株;有2株为EMB耐药菌株,但第306位点未见变化,与药敏试验吻合率为71.43%。结论:丽水市部分结核分枝杆菌embB基因第306位点突变与EMB耐药相关;总耐药率和总突变率偏低,可能还存在其它耐药机制。
Objective: To understand the mutation of embB gene in Mycobacterium tuberculosis resistant isolates in Lishui city. Methods: The mutations of embB gene in 47 clinical isolates of Mycobacterium tuberculosis and its resistance to EMB were analyzed by PCR-DNA direct sequencing. Results: There were 7 EMB-resistant TB strains in the susceptibility test, with a total resistance rate of 14.89% and an acquired resistance rate of 16.67%. No insertion or deletion of base was found at the 306 position of embB gene. Five strains had point mutations with a mutation rate of 10.64%, all of which were EMB-resistant strains. Two of them were EMB-resistant strains, but no changes were found at the 306th position. The coincidence rate with susceptibility test was 71.43%. CONCLUSION: The mutation of the 306th site of embB gene of some Mycobacterium tuberculosis in Lishui City is associated with EMB resistance. The total drug resistance rate and total mutation rate are low, and other drug resistance mechanisms may still exist.
