目的 探讨新生儿缺氧缺血性脑病 (HIE)血清半胱氨酸蛋白酶 1(caspase 1)、IL 1β、IL 18的变化及其与脑组织损伤程度的关系。方法 采用酶联免疫吸附法 (ELISA)检测HIE患儿 70例 (分轻、中、重 3组 )及正常对照组 2 2例新生儿d3血清cas pase 1、IL 1β和IL 18的水平。结果 1.与对照组比较 ,HIE各组血清caspase 1水平均显著增高 (P分别 <0 .0 5 ,<0 .0 1,<0 .0 1) ;HIE组间两两比较 ,中度与轻度组比较有显著差异 (P <0 .0 1) ;重度组与其他两组比较血清caspase 1水平均显著升高 (P <0 .0 1)。2 .HIE组血清IL 1β、IL 18与对照组比较 ,中、重度HIE组均显著增高 (P <0 .0 1) ;轻度HIE组与对照组无显著差异 (P >0 .0 5 )。HIE组间血清IL 1β、IL 18两两比较 ,中度与轻度组比较有显著差异 (P <0 .0 5 ) ;重度组与其他两组比较均显著升高 (P均 <0 .0 1)。 3 .血清caspase 1与IL 1β及IL 18均呈显著正相关 (γ =0 .793 4 P <0 .0 0 0 1;γ =0 .6677,P <0 .0 1)。结论 血清caspase 1、IL 1β和IL 18测定有助于HIE的早期诊断及临床分度。
Objective To investigate the changes of serum levels of caspase 1, IL-1β and IL-18 in neonates with hypoxic-ischemic encephalopathy (HIE) and their relationship with the severity of brain injury. Methods Serum caspase 1, IL-1β and IL-18 levels in 32 infants with HIE were detected by enzyme-linked immunosorbent assay (ELISA) in 70 neonates with mild, moderate and severe Hodgkin’s disease and in normal controls. Compared with the control group, the levels of serum caspase 1 in HIE groups were significantly increased (P <0.05, <0.01, <0.01) There was significant difference between mild group and control group (P <0.01). Serum caspase 1 level of severe group was significantly higher than that of the other two groups (P <0.01). Serum levels of IL-1β and IL-18 in HIE group were significantly higher than those in control group (P <0.01), but there was no significant difference between mild HIE group and control group (P> 0.05) . Serum levels of IL-1β and IL-18 in HIE group were significantly higher than those in moderate and mild groups (P <0.05), while those in severe group were significantly higher than those in other two groups (all P <0.05) 1). Serum caspase 1 was positively correlated with IL 1β and IL 18 (γ = 0.793 4 P <0.01 01; γ = 0.6677, P <0.01). Conclusion The serum levels of caspase 1, IL 1β and IL 18 are helpful for the early diagnosis and clinical index of HIE.
