目的利用电影磁共振成像(MRI)评估无明显心脏症状的2型糖尿病(DM)患者左心室功能变化。方法 85名2型DM患者和43名健康成年人行左心室短轴位电影MRI,计算并比较左心室整体收缩功能和血液动力学指标。统计学方法采用t检验、χ2检验。结果 2型DM患者与正常对照组相比收缩末期容积指数(ESVI)减低,射血分数(EF)增大,高峰充盈率(PFR)降低(均P<0.05)。结论心脏电影MRI能可靠评价无明显心脏症状2型DM患者的左心室功能改变。无明显心脏症状的2型DM患者发生舒张功能障碍早于收缩功能障碍。
Objective To evaluate left ventricular function in patients with type 2 diabetes mellitus (DM) without obvious cardiac symptoms using magnetic resonance imaging (MRI). Methods 85 patients with type 2 DM and 43 healthy adults underwent left ventricular short-axis cine MRI. The left ventricular systolic function and hemodynamic parameters were calculated and compared. Statistical methods using t test, χ2 test. Results Compared with the normal control group, the end-systolic volume index (ESVI), ejection fraction (EF) and peak-head filling rate (PFR) of type 2 DM patients were significantly decreased (all P <0.05). Conclusions Cardiac magnetic resonance imaging (MRI) can reliably evaluate left ventricular function in patients with type 2 diabetes mellitus without obvious cardiac symptoms. Diastolic dysfunction precedes systolic dysfunction in patients with type 2 DM without significant cardiac symptoms.
