目的:了解ICU中重症急性胰腺炎术后患者操作性疼痛体验及对疼痛管理的需求,为临床制定操作性疼痛管理方案提供依据。方法:采用目的抽样的方法,对在2019年5月1日—12月31日期间入住四川大学华西医院ICU 24 h内的15例重症急性胰腺炎术后患者进行半结构式深入访谈,采用Nvivo 10. 0软件进行资料分析。结果:通过资料分析得出以下6个主题:(1)患者经历了强烈的操作性疼痛;(2)患者对操作性疼痛的认识和应对不足;(3)患者的外源性支持不足;(4)外源性支持对患者很重要;(5)患者期待操作性疼痛有更好的管理方法;(6)医护人员对操作性疼痛的评估和认识不足。结论:重症急性胰腺炎术后患者在ICU住院期间经历了强烈的操作性疼痛,目前急需构建完善的操作性疼痛管理方案,为患者提供针对性的疼痛管理方案,改善ICU中重症胰腺炎术后患者操作性疼痛体验。“,”Objective:To understand procedural pain experience and need for pain management of ICU patients with severe acute pancreatitis after surgery, so as to provide basis for clinical formulation of operational pain management plan.Methods:Using the purposive sampling method, a semi-structured in-depth interview was conducted on 15 patients with severe acute pancreatitis who were admitted to ICU of West China Hospital of Sichuan University from May 1, 2019 to December 31, 2019. Nvivo 10. 0 software was used for data analysis.Results:Through data analysis, the following six themes were obtained: patients experienced strong procedural pain; patients' understanding and response to procedural pain was insufficient; patients' exogenous support was insufficient; exogenous support was important for patients; patients expected better management of operational pain; medical staff's assessment and understanding of procedural pain was insufficient.Conclusions:Postoperative patients with severe acute pancreatitis have experienced strong procedural pain during their stay in the ICU. At present, there is an urgent need to establish a complete procedural pain management plan, provide patients with targeted pain management plan and improve procedural pain experience of patients with severe pancreatitis after surgery in ICU.
