目的探讨腹腔镜下子宫腹壁悬吊固定术在女性盆底重建术中的应用价值。方法选取2015年10月-2016年10月该院收治的60例盆底功能障碍患者作为研究对象,均在腹腔镜下行子宫腹壁悬吊固定术,进行盆底重建,回顾性分析手术前后的盆底功能、生活质量等情况。结果手术时间平均为(152.56±55.87)min,术中出血量平均为(105.5±60.8)ml,尿管留置时间平均为(3.41±0.72)d,住院时间平均(4.21±1.11)d,术中及术后均未见明显并发症。60例患者均达到治愈标准,随访过程中未见复发。与术前相比,术后半年及术后1年用盆腔器官脱垂定量分度法(POP-Q)分度均明显降低(均P<0.05)。与术前相比,术后半年、术后1年的性功能问卷(PISQ-12)评分、排尿症状评分、排便症状评分、盆腔器官脱垂症状评分及盆底功能障碍问卷(PFDI-20)评分均显著降低(均P<0.05),并且术后1年的PISQ-12评分、排尿症状评分、排便症状评分、盆腔器官脱垂症状评分及PFDI-20评分均显著低于术后半年(均P<0.05)。结论腹腔镜下子宫腹壁悬吊固定术能够有效恢复盆底解剖学结构,改善盆底功能,治疗盆底功能障碍具有确切疗效。
Objective To investigate the value of laparoscopic hysterectomy for abdominal pelvic floor reconstruction in female pelvic floor reconstruction. Methods Sixty patients with pelvic floor dysfunction who were admitted to our hospital from October 2015 to October 2016 were selected as the study objects. All patients underwent laparoscopic hysteroscopic abdominal wall suspension fixation and pelvic floor reconstruction. The pelvis before and after surgery Bottom function, quality of life and so on. Compared with the preoperative, pelvic organ prolapse quantitative index (POP-Q) index was significantly decreased at 6 months and 1 year after operation (all P <0.05). PISQ-12 score, urination symptom score, defecation symptom score, pelvic organ prolapse symptom score and pelvic floor dysfunction questionnaire (PFDI-20) were compared with preoperative six months, postoperative one year, PISQ-12 score, urination symptom score, defecation symptom score, pelvic organ prolapse symptom score and PFDI-20 score at 1 year after operation were all significantly lower than those of 6 months after operation (both P <0.05) P <0.05). Conclusion Laparoscopic abdominal wall suspension fixation can effectively restore the pelvic floor anatomy, pelvic floor function, treatment of pelvic floor dysfunction with the exact effect.
