目的分析双水平无创正压通气(BiPAP)对慢性阻塞性肺疾病(COPD)合并呼吸衰竭的临床疗效。方法 40例COPD合并呼吸衰竭患者分为一般常规治疗组(20例)和BiPAP呼吸机治疗组(20例)。监测治疗前及治疗后2h、8h及48h动脉血气变化,观察患者血氧饱合度(SpO2)、心率(HR)及并发症的发生情况。结果 BiPAP治疗组治疗2h后,氧分压(PaO2)显著上升,与治疗前及常规治疗组同时段比较差异有统计学意义(P均<0.05),治疗后8、48h,pH值、SpO2及HR与治疗前比较差异有统计学意义(P均<0.05),且pH值、PaO2、二氧化碳分压(PaCO2)均优于同时段常规治疗组(P均<0.05)。结论 BiPAP能明显改善COPD合并呼吸衰竭患者的呼吸衰竭及全身情况。
Objective To analyze the clinical efficacy of bi-level non-invasive positive pressure ventilation (BiPAP) in patients with chronic obstructive pulmonary disease (COPD) complicated with respiratory failure. Methods Forty patients with COPD and respiratory failure were divided into general treatment group (20 cases) and BiPAP ventilator treatment group (20 cases). The change of arterial blood gas was monitored before treatment, 2h, 8h and 48h after treatment, and the occurrence of spO2, heart rate (HR) and complications were observed. Results After 2 hours of treatment, the partial pressure of oxygen (PaO2) in BiPAP group was significantly increased (P <0.05), and the values of pH, SpO2, There was significant difference between HR and before treatment (P <0.05), and pH, PaO2, PaCO2 were all better than those in the same period (all P <0.05). Conclusion BiPAP can significantly improve respiratory failure and systemic conditions in COPD patients with respiratory failure.
