目的 :用心率变异性 (HRV)观察肾脏病及肾功不全患者变化。方法 :选择肾功正常的肾病患者 2 6例 ,肾功不全患者 34例 ,正常对照组 34例。用 2 4h动态心电图分析其 HRV的时域指标。结果 :肾功正常肾病组平均 RR间距期 (RR I)、2 4h窦性 R- R间期标准差 (SD)、5 min窦性 R- R间期标准差 (SDAIDX)显著低于正常组 (P<0 .0 5 ) ;肾功不全组 6项指标均明显低于正常对照组 ,SD,5 min窦性 R- R间期标准差的均值 (SDIDX) ,SDAIDX,及 R- R连续差异均方根 (r MSSD)显著低于肾功正常组。结论 :肾脏病患者在肾功正常期即有自主神经功能损伤 ,以交感神经活动受损主为 ;肾功不全患者交感神经及迷走神经功能均受损
Objective: To observe the changes of patients with renal disease and renal insufficiency with heart rate variability (HRV). Methods: Twenty-six patients with normal renal function, 34 patients with renal insufficiency and 34 normal controls were enrolled. The 24 h ambulatory electrocardiogram was used to analyze the time domain index of HRV. Results: Mean RR interval (RR I), standard deviation of sinus R-R interval (SD), and standard deviation of sinus R-R interval of 5 min (SDAIDX) in normal renal function group were significantly lower than those in normal group (P <0.05). The 6 indexes of renal insufficiency group were significantly lower than that of the normal control group, SD SDXX, SDAIDX, and R-R continuous The difference root mean square (r MSSD) was significantly lower than the normal renal function group. CONCLUSIONS: Nephropathy patients have autonomic nerve function impairment during normal kidney function, and sympathetic nerve activity is mainly damaged. Sympathetic and vagal nerve functions are impaired in patients with renal insufficiency
