目的探讨在上海社区卫生服务中心开展临床预防服务时,影响医患双方互动的有利和阻碍因素。方法本研究采用定性研究的方法,对象为社区居民、社区临床工作者和管理者。结果开展临床预防工作的主要障碍在医患双方都存在。社区临床医生人力短缺比较严重;工作量过大;自身专业水平仍需提高。而居民方面人口文化层次高低不一;缺乏对社区卫生中心及其工作人员信任和理解;中青年社区居民多由于工作而较难在时间上和社区的卫生服务形成交集。结论目前在社区卫生服务中心临床预防服务还处于一种医务人员半自发地提供相关服务的状态,需要从人力、物力、财力方面加大投入。
Objective To explore the favorable and obstructive factors that affect the interaction between doctors and patients when implementing clinical prevention services in Shanghai community health service centers. Methods This study used qualitative research methods for community residents, community clinicians and managers. Results The main obstacles to carry out clinical prevention exist in both doctors and patients. Community clinicians are more serious shortage of manpower; workload is too large; their professional level still needs to be improved. However, the level of population and cultural level of residents is not the same; the trust and understanding of community health center and its staff are lacking; young and middle-aged community residents are more likely to encounter the community health service due to their work. Conclusion At present, the clinical prevention service in the community health service center is still in a condition that the medical staffs provide relevant services semi-spontaneously and requires more manpower, material resources and financial resources.
