血细胞体外培养和克隆化技术的成功和应用,为深入研究白血病细胞增殖、分化的调控机制打开了大门。尽管白血病细胞分化停滞的真正机制仍不清楚,但实验表明,在某些诱导剂的作用下,某些类型的动物和人类白血病细胞可以继续分化、成熟,发展成有功能的粒细胞和巨噬细胞。在这些体外实验的基础上,发展了白血病的诱导分化治疗的概念。本文回顾了近年来有关文献,就诱导分化治疗理论的实验基础、临床意义、未来展望等问题进行了复习,重点介绍了小剂量 Ara-c 的疗效、适应症、副作用及其机制探讨。
The success and application of blood cells in vitro culture and cloning technology opens the door for further research on the regulatory mechanism of leukemia cell proliferation and differentiation. Although the exact mechanism of leukemic cell differentiation arrest remains unclear, experiments show that certain types of animal and human leukemia cells can continue to differentiate, mature, and develop into functional granulocytes and macrophages under the action of certain inducers cell. Based on these in vitro experiments, the concept of differentiation-induced leukemia has been developed. In this review, we review the literature in recent years and review the experimental basis, clinical significance, and future prospects of the theory of differentiation-induced therapy. The clinical efficacy, indications, side effects and mechanisms of low-dose Ara-c treatment are reviewed.
