目的:探讨乳腺叶状肿瘤声像图及钼钯特征与其病理基础,旨在提高其诊断水平。方法:回顾分析18例经手术病理证实的乳腺叶状肿瘤患者的彩色多普勒超声和钼靶X线表现。观察其超声图像和钼靶影像图特点,并与病理对照。结果:具有一定特征的声像图表现为病灶多呈圆形、椭圆形或不规则形,轮廓多呈分叶状,无包膜回声,但病灶与周围组织界限较为清晰,呈高低混合回声、分布不均匀、有线条状高回声间隔。有散在分布的大小、形状不一的无回声区或低回声区。钼钯X线影像图表现:病灶多表现为密度均匀、边缘光滑的分叶状肿块。其上述特征相应的病理基础为癌组织体积较大,呈圆形或类圆形,可见肉瘤性间质成分和良性上皮成分,有裂隙形成,有出血、坏死、囊性变。结论:超声联合钼靶X线对乳腺叶状肿瘤的定性诊断具有较高的临床应用价值。它们的图像特征是有病理基础可循的。
OBJECTIVE: To investigate the sonographic features of breast phyllodes tumor, the characteristics of molybdenum-palladium and its pathological basis, in order to improve its diagnostic level. Methods: The color Doppler ultrasonography and mammography X-ray findings of 18 patients with phyllodes tumor confirmed by surgery and pathology were retrospectively analyzed. Observe the characteristics of ultrasound images and mammography images, and with the pathological control. Results: The sonogram with certain characteristics showed that the lesions were mostly round, oval or irregular. The outline was lobulated and had no echogenic echo. However, the boundary between the lesion and the surrounding tissue was clearer, with a mixture of high and low echoes. Uneven distribution, high-echo line spacing. There are scattered size, shape of the echo-free area or hypoechoic area. Molybdenum Palladium X-ray image performance: lesions showed more uniform density, smooth edge lobulated mass. The corresponding pathological basis of the above features is that the cancerous tissue is relatively large in volume and is round or round in shape, showing sarcomatoid interstitial components and benign epithelial components, with the formation of fissures, bleeding, necrosis and cystic degeneration. Conclusion: Ultrasound combined with molybdenum target X-ray diagnosis of breast phyllodes tumor has a high clinical value. Their image features are pathologically based.
