目的 应用彩色多普勒超声心动图 (CDFI)比较经导管五种方法堵闭小儿动脉导管未闭 (PDA)的疗效。方法 1989年 3月~ 1999年 7月 ,超声心动图分别观察采用海绵塞 (Porstman)、双面伞堵闭器 (Rashkind)、纽扣式补片(Sideris)、弹簧圈堵闭 (Coil)和蘑菇伞堵闭塞 (Amplatzer)堵闭2 82例PDA患儿术前、术后 1天、3个月、6个月大动脉水平有无分流、心脏大小变化、堵闭器的位置及其血流动力学的改变。结果 5组患儿术后 1天左室舒张末径 (LVDD)和左房径 (LA)明显缩小 (P <0 0 0 1) ,6个月基本恢复正常 ;术后 1天残余分流 17%、3个月下降至 6 4%。 6个月无进一步下降。结论 Amplatzer和Coil堵闭法分别堵闭对小于或等于8 1mm和 2 5mm的PDA近远期疗效确切 ,已基本能替代手术结扎的方法 ,值得在临床推广
Objective To compare the efficacy of five methods of closure of pediatric patent ductus arteriosus (PDA) by color Doppler echocardiography (CDFI). Methods From March 1989 to July 1999, echocardiography was performed using Porstman, Rashkind, Sideris, Coil, and mushrooms Amplatzer occlusion 2 82 PDA patients with preoperative, postoperative 1 day, 3 months, 6 months with or without aortic shunt, heart size changes, the location of the shutters and hemodynamics Change. Results The left ventricular diastolic diameter (LVDD) and left atrium diameter (LA) of the 5 groups were significantly reduced (P 0 001) at 1 day after operation and returned to normal after 6 months. The residual shunt was 17% , 3 months down to 64%. No further decline in 6 months. Conclusions Amplatzer and Coil blockade methods are effective in short-term and long-term follow-up of PDA less than or equal to 8 1mm and 2 5mm, respectively. They have basically replaced the surgical ligation and are worthy of clinical promotion
