目的:对腓骨+不同钛植入体的上颌骨重建的三维有限元模型进行生物力学分析,研究上颌骨功能性重建中用钛植入体重建颧上颌支柱的可行性。方法:分别对腓骨+重建钛板、腓骨+钛网和腓骨+颧种植体3种上颌骨重建的三维有限元模型,于双侧后牙区垂直加载,研究钛植入体的应力和位移分布。结果:腓骨+重建钛板和腓骨+钛网这2种类型在钛板和钛网的转折处应力和位移最大;腓骨+颧种植体在颧种植体植入颧骨处的应力最大,在颧种植体的中点位移最大。结论:在上颌骨功能性重建中,用颧种植体重建颧上颌支柱是可行的,而用钛板或钛网重建颧上颌支柱则不可行。
OBJECTIVE: To investigate the feasibility of reconstructing zygomatic maxillary struts with titanium implants in functional reconstruction of the maxilla with three-dimensional finite element models of maxillary reconstruction of fibula + different titanium implants. Methods: Three kinds of maxillary reconstruction models of fibula + reconstruction titanium, fibula + titanium mesh and fibula + zygomatic implants were made and loaded vertically in bilateral posterior teeth to study the stress and displacement distribution of titanium implants . Results: The stress and displacement of fibula + reconstructed titanium plate and fibula + titanium mesh were the highest at the turning point of titanium plate and titanium mesh. The stress of the fibula + zygomatic implants was maximal at zygomatic implants, The mid-point displacement of the implant is the largest. Conclusion: It is feasible to reconstruct the zygomatic maxilla with zygomatic implants in functional reconstruction of the maxilla. However, reconstruction of zygomatic maxilla with titanium plate or titanium mesh is not feasible.
