Objectives The treatment of complex post-traumatic calcaneal malunion is very difficult , the objective of this study was to investigate the clinical results of the subtalar joint distraction arthrodesis with “double-wedge bone block grafting”. Methods From April 2004 to December 2007,twentysix patients with Stephen type-Ⅲpost-traumatic calcaneal malunions were treated with subtalar joint arthrodesis with “double-wedge grafting”.We preformed lateral exostectomy and a peroneal tenolysis,a subtar joint debridment and fusion with a tricorticaliliac autograft(in which has a configuration of narrow-anterior wide-posterior and narrow-lateral wide-medial)to correct hindfoot deformity,and stabilization with two to three titanium cannulated lag screws.They were evaluated with use of AOFAS Ankle-Hindfoot scale.Radiographs were evaluated for the talar-calcaneal angle and talar-1st metatarsal angle.Results Twenty-two patients were hindfoot varus malalignments,at an average of 18.2 months(12-29 months) postoperatively,the hindfoot malalignments of twenty-three patients were corrected significantly, the pain of twenty-two patients were reduced or relieved significantly. All of the fusion sites were united.Complications included 3 wound dehiscence which resolved after short term dressing changes.The average AOFAS ankle hindfoot scale improved from 33.6 (20-49)pre-operatively to 81.3(56-92)postoperatively. And the average pain score improved from 7.8(7-9)pre-operatively to 2.1(0-4)postoperatively. The talar-1st metatarsal angle also improved from 17.4(14-24) pre-operatively to 6.1(4-12) post-operatively.Conclusions The subtalar joint arthrodesis with “double-wedge grafting” can correct the calcaneal malalignment and reduce the pain without use of complex displacement calcaneal osteotomy.It’s a good alternative for the treatment of the post-traumatic calcaneal malunion.
