Objective To present the result of our initial experience in using the self-expanded stent assist coiling of the large and giant intracranial wide.necked aneurysms.Methods A total of 27 patients with large or gaint wide-neck intracranial aneurysms were treated with stent-assisted technique.Our operation fashions include stent-jailing technique, stent 缸 ket technique, Y-configuration double— stent technique, kissing stent technique and waffle cone technique et al.Results : Coil embolization and covered stent placement were technically successful in all patients.theinitial angiographic results showed complete occlusion in 16 patients(59.3%), incomplete occlusion in 1 I patients.Clinical follow-up Was available in all patients at 3 ~ 9 months.In 4 patients the aneurysms were recurrent and had retreatment with detachable coils.A of the patients' symptoms such as headache, visual field deficit and oculomotor dysfunction were resolved or improved.Conclusion : Stent-assisted coil embolization is a new tool in the management of intraeraniai aneurysm and maybe particularly useful in the case of wide-neck and large aneurysm.The stent serve as a mechanical scaffold for the placement of aneurysm coils, prevents coil protrusion into the parent artery and may allow safer packing of the aneurysm with a denser coil mesh.
